• Patient Care: Deliver high-quality, compassionate care to residents under the direction of the Clinical Care Manager, following LPN duties and responsibilities.
• Medication Administration: Administer medications and treatments as prescribed by the physician (oral, intravenous, injection, rectal, and topical routes).
• Clinical Observations: Monitor residents’ conditions, identify changes, and report findings to the attending physician and Clinical Care Manager.
• Specimen Collection: Collect specimens for laboratory testing, ensuring proper storage and timely notification.
• Record Keeping: Maintain accurate and up-to-date records of residents’ care and treatment in the electronic health record (EHR) system.
• Collaboration & Support: Communicate with residents, families, and the interdisciplinary team in a professional manner, providing emotional support when necessary.
• Daily Living Assistance: Help residents with daily activities, such as feeding, hygiene, and mobility, to promote comfort and independence.
• Resident Assistance Management: Supervise Resident Assistants, providing them with detailed reports and updating care plans as necessary.
